DISABILITY SUPPORT WORKER -Mitcham

JOB DESCRIPTION: The Disability Support Worker is responsible for providing support services aimed at maximising the living, employment, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs. 
TASKS & RESPONSIBILITIES:Support Clients to meet their NDIS goals.
Encourage and assist Clients with daily activities, home care, personal care as required
Effective communication skills
Ability to work independently and effectively manage time
Demonstrate understanding of a person-centred approach
Follow established support work practices
Effective verbal and written communication skills and the ability to connect with participants
Promote and assist Clients to live an active, healthy and positive lifestyle
Ability to follow individual support care plans
Availability to work various shifts Monday to Sunday
Skill Development – Shopping, cooking, budgeting, using public transport
Transport Clients as required.
Community Access – Assist clients to engage with the community by taking the participant to various events and activities in the community
Any other reasonable duties as required
Follow all organisational policies and procedures.

REPORT TO:  Manager
LOCATION:  Mitcham
QUALIFICATIONS/ REQUIREMENTS: Cert III Individual Support/ Cert Mental Health/ Cert in Aged Care/ Cert in Community Services/ Cert Disability Work or similar.
Current WWCC
Criminal Record Check or willing to undergo one
First Aid Certificate
Driver’s License (Reliable and comprehensively insured vehicle) Smart phone 
Minimum 6 months relevant experience.
 SKILLS:Excellent verbal and written communication skills. Ability to build rapport and trust with clients. Strong time management and ability to priorities. Ability to use Microsoft Office suite of programs and other computer systems for record keeping. Excellent problem solving and conflict management skills. Ability to work as part of a team and maintain good working relationships.
